Consent for Online Consultation

By booking, you confirm that

• You understand this is an online consultation and the dentist cannot physically examine you
• You consent to sharing your symptoms, photographs and records with the consulting dentist
• You will seek in-person care if the dentist advises it, and emergency care if you have emergency signs
• For a minor, you are the parent or legal guardian and will be present during the consultation

Withdrawing consent

You may withdraw consent at any time by contacting contact@askmydentist.in. This does not affect care already provided.
